College Scholarship Award

Minium Amount of Scholarship awarded: $1,000.00

Our scholarship fundraising goal for May of 2009 is $51,000.00
$1,000.00 per scholarship per state in the United States including Puerto Rico

Read below for details on how to nominate a scholarship winner for 2009




Our scholarship will be awarded to a senior High School Athlete

who is fighting cancer but will be attending college the

Fall following their High School graduation

or

a senior Athlete who has a parent fighting the battle of cancer. 

 

The Athlete will be nominated by his/her High School Coach 

The coach will write a letter of nomination including the following....

 

1. A general history of the Athlete as he/she relates to the team

2.  The impact the Athlete has had on the team

3.  The impact the Athlete has made on you, the coach.

4.   If the Athlete has cancer, explain the Athlete's specific type cancer, general overview of treatments and present health status.  If the Athlete's parent is fighting cancer, give a general overview of their situation.

5. Please give us any other information you would like to share about the player and why he/she should recieve the scholarship award.

 

Coaches Please Note: Nominations must be sent from your school e-mail address.

 

The 4th annual Champion of Life College Scholarship Award will be awarded to the Athlete no later than May 31, 2009. 

Deadline for Nominations:  April 30, 2009

The Scholarship Winner's Coach will be notified on May 15, 2009
